ACTIVITY REGISTRATION

If you would like to get registered for one of our programs or activities, please read the following steps to help you in the process of making sure you get us all necessary forms. On this page you will find steps for registering for:

Individual Recreational Activities
Ability Assessments
Private Lessons
Rental Equipment

At this time we do not have the means to provide registration via an online system. You can download a PDF version of most of our materials OR feel free to request to have materials sent to you.

If you need help with translation of materials or need help filling out any of the forms, please contact our office to talk to a staff member. We can help you fill it out over the phone if you are needing assistance or if you are visual impaired and cannot translate the PDF document. Starting in Summer 2010 we will have our guides available in PDF and also in a Word format for easier translation. We apologize for the inconvenience at this time.
